The Accreditation Application Amendment Form facilitates changes to information submitted on the organization's application for accreditation.
 
The Competency Offering Amendment Formseeks to solicit information re: organization's intention for additions or removal of units of competencies or qualifications offered 

The Annual Report Form gives the organization a guide as to the information required for the annual review and should be submitted thirty (30) days after the organization’s accreditation anniversary date.
